ADMISSION PROCESS

We are very excited that you have made the decision to join our School Family. There are a few forms that we need you to complete in order to register your family with us. Below you will find links to our enrollment packet, and forms that are specific to various age groups. Please let us know if you have any questions!

Enrollment Form

Each child will need their own enrollment form completed.

Child's Pre-Admission Record

Each child will need their own Admission Record completed.

Infant Feeding Schedule

This form must be completed by the parent for each infant under the age of 18 months.

Pediatrician Permissions Form

This form must be downloaded and completed by your child’s healthcare provider prior to their first day in care. Each child needs their own form completed.

New Family Orientation

Each family is required to review the New Family Orientation information. This essential information will help you prepare for enrollment and provide insights into what you can expect during your journey with us.
